Youth Unlimited's Targeted Case Management provider works to ensure that each child who has therapeutic needs are linked to the therapeutic services appropriate to best meet their mental health needs. Working with the family or custodian, the Targeted Case Manager arrange diagnositc assessments, creates and maintains a Person-Centered Plan for each client and closely monitors and evaluates treatment options and arranges services throughout his or her time with Youth Unlimited. The Targeted Case Manager will attend monthly treatment team meetings to stay up to date on each child's progress and needs.
